We strongly recommend new patients contact their carrier prior to making an appointment at Bella to confirm the details of your plan.
We will file claims with insurance carriers on your behalf.
Patients are responsible for paying any co-pays, deductibles, or outstanding balances prior to the time of service.
Self-Pay & Health Share Plans
SELF PAY | Many of our patients are self-pay. We partner in your care by providing generous savings off listed medical service rates. Anticipated fees and outstanding balances are collected at time of service.
HEALTH SHARE | We are pleased to accept patients with health share plans. Health share plan patients are considered self-pay at Bella. Patients enjoy generous savings on all services. Payment is collected at the time of service. We do not file claims on your behalf.
Medicaid & Medicare
MEDICAID | We are not accepting new Medicaid patients at this time. OB exceptions. Call for details.
MEDICARE | We are not accepting new Medicare patients at this time.
GOOD FAITH ESTIMATES (GFEs)
GOOD FAITH ESTIMATES | All patients paying in cash for medical care - whether self-pay, participating in a share plan, or insured but choosing to self-pay - are welcome to request a Good Faith Estimate (GFE) from Bella for services at the time of scheduling (or at any time prior to your care).
GFEs automatically will be provided for all patients scheduling PROCEDURES at Bella. (The only exception to this is hormone replacement therapy. It is recurring care and will be made available only upon request.)
If a GFE request is made (or a procedure is scheduled) at least ten days in advance, Bella will provide the GFE no later than three business days (M-F) after the date of scheduling.
If a GFE request is made (or a procedure is scheduled) fewer than ten days in advance, Bella will provide the GFE one business day (M-F) after the date of scheduling.
Financial Assistance Program
Bella Health + Wellness Financial Assistance Program offers discounts or free care to those who financially qualify.
