FUNCTIONAL MEDICINE PAYMENT INFORMATION

DISCOVERY CALL & FIRST VISIT

Bella is a unique provider in the Functional Medicine world. As a primary care provider with contracts with most major insurance companies (see list), we are able to bill patients’ insurance plans for some Functional Medicine costs.

But insurance does not cover all of the costs of the comprehensive physical examination, medical history, lab panels, and diagnostics involved with initiating therapy. Patients will also incur some out-of-pocket expenses (both the insured and those who self-pay.)

Patients prepay the Investigative & Consultation Fee before beginning Functional Medicine treatment at Bella.

The fee will be paid after your free discovery call (to ensure you’re a good candidate for the program), but before your first appointment with a Bella Functional Medicine provider. (Patient will not be scheduled until fee is paid.)

IMPORTANT: The investigative fee is non-refundable. Patients can be rescheduled at no charge if they offer at least 72 business hours’ notice. Patients must reschedule within 12 months of the original appointment date.

FOLLOW UP VISITS

On subsequent visits, patients will simply pay the normal fees incurred with care. For those with insurance, we will bill your carrier as normal. For those who are self-pay, you will pay the typical out-of-pocket expenses.
